About the Role

LHC Group is seeking a skilled Physical Therapist to join our Home Health team in Belmont, Massachusetts. As a Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes, helping them to regain their independence and improve their overall well-being.

Key Responsibilities
  • Evaluate patients' functional status and physical therapy needs, working closely with the physician to develop a personalized therapy plan.
  • Develop and implement treatment plans to improve patients' mobility, endurance, and overall physical function.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and their caregivers to promote optimal health outcomes.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a valid driver's license and access to a reliable vehicle.
