Job Title: Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team at Caregiver, Inc. As a key member of our healthcare team, you will play a vital role in providing exceptional care and support to individuals with intellectual and developmental disabilities.

Responsibilities:
  • Provide direct and individualized nursing care to consumers, including those receiving Wellness Coordination Services.
  • Develop, document, implement, and evaluate health care protocols and nursing care plans for each consumer.
  • Ensure the prescribed medications and treatments are available in the home for each consumer.
  • Act as a resource for agency LPNs and ensure the monitoring of vital signs, weight charts, immunization records, and health status of DSA consumers.
  • Participate in Area Individual Support Team (IST) meetings and share knowledge, information, and ideas among team members.
  • Record information concisely, accurately, and completely, and ensure consumer medical information charts are current.
  • Maintain a positive working relationship with family members and guardians of DSA consumers.
  • Assist in the development of staff training curriculums, including training in detecting signs of illness or dysfunction that warrant medical or nursing intervention.
  • Administer and read TB skin tests as necessary for consumers and staff.
  • Ensure nursing assessments are completed per agency policy.
  • Participate in the nursing on-call schedule and serve as a DSA, Inc. representative to trade organizations and state or regional committees.
Qualifications and Skills:
  • Degree from an accredited school of nursing.
  • Current state licensure as a licensed nurse.
  • Five years of clinical practice preferred.
  • BSN preferred.
  • One year of experience working with individuals with developmental disabilities or in a related field preferred.
  • Maintains credentials necessary to teach Core A/B courses.
  • Maintains certification necessary to administer and read tuberculosis skin tests.
  • Ability to communicate effectively; orally and in writing.
  • Ability to use common word-processing, spreadsheet, and communications software programs within a Windows environment.
