Physician Assistant/Nurse Practitioner

Job Summary:
We are seeking a highly skilled Physician Assistant/Nurse Practitioner to join our team at Trillium Health NY. As a key member of our healthcare team, you will provide comprehensive care to our patients, including primary and specialty care, education, and health promotion.
Key Responsibilities:
- Provide primary and specialty care to patients, including histories and physicals, routine and urgent office visits, and review of laboratory and test results.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Participate in health education and promotion activities to empower patients to take an active role in their healthcare.
- Conduct regular peer reviews and focused chart reviews to ensure high-quality care and adherence to agency policies.
Requirements:
- Minimum 3 years' experience as a board-certified Nurse Practitioner or Physician Assistant.
- M.S. degree in a National League for Nursing (NLN) accredited school of nursing or M.S. degree in Physician Assistant studies.
- Current licensure in New York State as a Registered Professional Nurse and current certification in New York State as a nurse practitioner or Physician Assistant.
- National certification by the American Nurses' Association (ANA) as a nurse practitioner (adult or family, preferred).
Preferred Qualifications:
- Experience working in community-based health care settings.
- Understanding of EMRs.
- Ability to demonstrate teamwork and effective interpersonal relations skills.
Physical Requirements:
The employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
