Clinical Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessor. In this role, you will conduct comprehensive health assessments of older adults and other at-risk individuals in their homes, identifying potential health risks and providing recommendations for improvement.
Key Responsibilities:
- Conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits
Requirements:
- Master's degree required or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP/PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner/Physician Assistant, with the ability to travel in-state or out-of-state
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills with patients, clients, and colleagues
- A positive and outgoing personality, with a willingness to work collaboratively as part of a team
