Family Support Case Coordinator

The Child Welfare Case Manager (CWCM) plays a crucial role in supporting families referred by the Department of Children and Families (DCF). The primary objective of the CWCM is to evaluate and address the needs of clients and their families stemming from allegations of abuse or neglect, aiming for a stable and permanent resolution. Under the guidance of the CWCM Supervisor, the CWCM is tasked with various responsibilities including service coordination, assessment completion, report preparation, and providing essential support to families.
Key Responsibilities- Participate in the Phase I Child Welfare Practice Pre-service Training program
- Perform child safety evaluations and formulate safety plans
- Oversee service provision and make necessary referrals
- Maintain comprehensive case records and generate reports
- Assist in placement processes and promote independent living
- Engage in the Quality Improvement initiatives
Educational Background:
- Bachelor's Degree or equivalent professional experience
- Associate degree or 60+ college credits with a minimum of two years of relevant work experience
- High school diploma or GED with four or more years of experience in social work or human services
Licenses and Certifications:
- Valid driver's license
- Current car insurance
