We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care and services for our residents.
Key Responsibilities- Administer medications and treatments in a safe and organized manner, adhering to federal, state, and local laws, as well as Sunrise Senior Living policies.
- Document medication administration, conduct SHUs, and provide resident care while demonstrating the Sunrise Senior Living Mission.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates, ensuring that each resident receives personalized care.
- Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure accurate medication administration and updates.
- Conduct audits of medication carts and maintain the Medication Room, medication carts, and treatment carts for neatness, cleanliness, and availability of medications.
- Follow re-fill processes for medications and maintain compliance with Sunrise Senior Living policies and procedures.
- LPN, LVN, or state/province-specific licensed nurse credential.
- Certifications in CPR and First Aid.
- Ability to make choices and decisions in the best interest of residents.
- Ability to react and remain calm in difficult situations.
- Ability to handle multiple priorities and possess written and verbal skills for effective communication.
