We are seeking a skilled Hospice Nurse Practitioner to join our team at Gentiva Hospice. As a key member of our interdisciplinary team, you will be responsible for conducting Face-to-Face encounters with patients and gathering clinical information to support hospice diagnosis and prognosis.
Key Responsibilities- Perform physical assessments for F2F encounters within regulatory time frames
- Provide clinical findings to physicians for recertification of patient eligibility
- Make recommendations to the IDG based on assessment findings
- Notify the hospice agency of any scheduling issues
- Master's or higher degree in Nursing
- Graduate-level nurse practitioner education program accreditation
- Current state license as a registered nurse
- Current state license or certificate as an advanced practice nurse
- Current national certification as a Nurse Practitioner (ANCC or AANP)
- Collaborative agreement in place
- Comprehensive benefits package, including health insurance and 401k plan
