Job Summary

We are seeking a skilled RN Clinical Liaison to join our team. As a key member of our hospice care team, you will be responsible for educating healthcare professionals about our services and providing end-of-life solutions that are committed to clinical excellence, compassion, and dignity.

Key Responsibilities
  • Ensure the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
  • Complete the admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions.
  • Provide accurate and timely information regarding hospice services to anyone inquiring.
  • Receive calls from referral sources, respond timely, and initiate paperwork to begin the intake/referral process.
  • Serve as a patient advocate in evaluating/developing a plan of care and obtaining informed consent for hospice.
  • Call on multiple account types, including SNFs, ALFs, hospitals, and physicians.
  • Offer in-services and education on the benefits of hospice.
Requirements
  • A compassionate and empathetic approach to patient care.
  • A valid RN license in the state(s) you will be employed.
  • A minimum of 2 years of healthcare experience, preferably in hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
  • Knowledge and respect within your community.
