Job Summary

As a key member of our team, the Health and Wellness Program Manager will oversee the implementation, coordination, and supervision of a comprehensive Health, Wellness, and Clinical Program that addresses the needs of all Assisted Living Residents. This role requires a strong understanding of nursing practices, geriatric care, and community health.

Responsibilities
  • Develop and implement a Wellness Program that includes weekly scheduled resident Health and Wellness Check-ups to ensure a proactive and preventative clinical approach.
  • Provide nursing assessments, care planning, care oversight, and quality assurance for all residents.
  • Maintain accurate and up-to-date resident charts, including progress notes, identification of nursing interventions, treatments, and assessments.
  • Coordinate services of outside agencies providing healthcare or other assistance to residents, including home health agencies, physical therapists, hospice, laboratories, durable medical equipment suppliers, and other professional services.
  • Review and sign-off on Incident Reports with the Executive Director and Assistant Director, establishing intervention and documentation of follow-up.
  • Attend classes, seminars, and meetings to maintain a high level of knowledge regarding current nursing theory, trends, and regulatory changes.
  • Perform other duties as assigned.
Requirements
  • Must be a graduate of an accredited school of nursing with three years of experience in nursing, preferably in geriatric services.
  • Must hold a current Maryland license as a registered professional nurse.
  • Baccalaureate degree in nursing or related health field with geriatric emphasis preferred.
  • Must have successfully completed the 20-Hour Delegating Nurse Course within 60 days of employment.
