Alluvion Health seeks a skilled and compassionate Nurse Care Specialist to provide comprehensive nursing services to individuals and families in our community.


Key Responsibilities:
  • Coordinate patient care with healthcare providers and care teams, ensuring seamless transitions and optimal outcomes.
  • Review and update medical records before patient visits, maintaining accurate and up-to-date information.
  • Assess immunization status and administer treatments, medications, and immunizations as ordered by healthcare providers.
  • Prepare patients for physical exams and procedures, providing education and support throughout the process.
  • Facilitate referrals to community providers and organizations, promoting continuity of care.
  • Document patient encounters, treatments, and follow-ups in our electronic medical record system.

Requirements:
  • A RN degree is required; a BSN is preferred.
  • 3-5 years of experience in primary healthcare or equivalent.
  • A valid Montana Nursing License and driver's license are required.

Compensation:

The estimated annual salary for this position is $65,000-$85,000.
