Position Summary:
The Outreach Worker plays a crucial role in promoting housing stability initiatives within the community, specifically targeting low-income families who are at risk of homelessness or currently experiencing homelessness. This position involves extensive community engagement to establish resources that support families in need of housing solutions. The Outreach Worker is part of a collaborative team that provides comprehensive case management, housing referrals, financial guidance, and benefits assistance.
Key Responsibilities:
- Conducts outreach activities, providing engagement, crisis intervention, and trauma-informed case management for clients and their families.
- Performs initial assessments and identifies immediate needs for individuals in crisis.
- Collaborates with the case management team to offer information referrals, crisis support, and assistance with securing permanent housing.
- Assists in the comprehensive evaluation of clients to gather relevant information for developing personalized service plans.
- Coordinates follow-up services with the case management team and meticulously documents all interactions and services provided.
- May perform duties of a Case Manager or Housing Specialist as required.
- Supports program staff in implementing effective strategies and practices to deliver holistic services, including life skills training.
- Facilitates workshops addressing household needs and provides crisis intervention as necessary.
- Represents Volunteers of America at community meetings focused on housing solutions for homeless veterans and develops partnerships with local organizations.
- Performs additional responsibilities as assigned.
Qualifications:
Requirements:
- Must successfully pass a fingerprint clearance and background check, including criminal history and employment verification.
Education:
- Associate's Degree in Social Services or a related field, or an equivalent combination of education and experience (two years).
Experience:
- Minimum of one year of experience working with veterans, individuals in recovery, or those experiencing homelessness.
Preferred Qualifications:
- Bilingual in Spanish.
- Familiarity with HMIS (Homeless Management Information System).
