Job Description:
This Case Management Professional role involves assessing, planning, coordinating, and evaluating services to meet the needs of our members with high need potential. You will work remotely in the beautiful state of New Mexico and collaborate with healthcare teams to deliver exceptional patient care. The ideal candidate has 1-2 years of experience in case management or a related field and is passionate about making a real difference in the lives of others.

Responsibilities:
  • Conduct clinical assessments and determine eligibility for case management based on clinical judgment.
  • Develop and implement personalized care plans in collaboration with healthcare teams.
  • Monitor care plans, evaluate effectiveness, and suggest changes as needed.
  • Promote integration of services, including behavioral health care and long-term services and supports.

Requirements:
  • Bachelor's or Master's Degree in social science, psychology, gerontology, public health, or social work (or related field).
  • 1-2 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
  • Valid driver's license with good driving record and reliable transportation.
