About the Role

We are seeking a highly skilled and compassionate Clinical Coordinator RN to join our team at DaVita. As a Clinical Coordinator RN, you will play a critical role in delivering exceptional patient care and services in our outpatient dialysis center.

Key Responsibilities
  • Coordinate ESRD patient care and services, ensuring the highest level of care and safety for our patients.
  • Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
  • Monitor and track patient metrics, including lab values, vital signs, and outcome management.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience, preferably in dialysis or nephrology.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification preferred.
  • Basic computer skills and proficiency in MS Word and Outlook required.
