Job Summary

We are seeking a skilled RN Case Manager to support our Director of Health and Wellbeing in the day-to-day clinical operations of our community. The successful candidate will plan, organize, and coordinate the nursing process to provide exceptional care to our residents, utilizing a person-centered and holistic approach.

Key Responsibilities
  • Manage resident care with specific knowledge and experience in assessments, symptom management, crisis intervention, service recovery, and family collaboration.
  • Collaborate with other healthcare entities, such as providers or therapeutic disciplines, in the assessment of resident care needs and adjust the resident service plan to correlate with best-recommended care.
  • Identify clinical education needs and conduct comprehensive clinical assessments to promote successful outcomes.
  • Assign and oversee appropriate delegation of services to subordinate team members.
  • Maintain clinical data integrity through best practice documentation standards and appropriate use of data-based systems.
  • Respond immediately to unexpected/emergency situations, including emergency response system calls from residents, security issues, and falls.
Requirements
  • Graduate of an accredited school of nursing.
  • Licensed as a Registered Nurse (RN) in good standing by the state in which the community is located.
  • Preferred two years of general nursing experience within a geriatric setting.
  • Dementia certification or willingness to become certified.
  • Preferred previous supervisory and leadership experience.
