Salary

New Jersey Residents Only: The estimated annual salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including local labor markets, education, work experience, certifications, etc.

Job Description

In this role, you will have the ability to achieve work life balance. Flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends, and no holidays required.

You will be responsible for conducting comprehensive assessments, managing members through acute medical situations or exacerbations of chronic conditions, and ensuring accurate and complete documentation.

You'll also be responsible for communicating and collaborating with the interdisciplinary care team, conducting advanced illness and advanced care planning conversations, and providing patients and caregivers with counseling and education.

Required Skills and Qualifications

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted APC license in the State of New Jersey, or the ability to obtain by start date
  • Active Nurse Practitioner certification through a national board
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment
  • Access to reliable transportation
