Job Summary:
Arosa is seeking a highly skilled and experienced RN Care Manager to join our team. As a key member of our care team, you will be responsible for planning and coordinating all services provided to clients, serving as the clinical expert to all staff members assigned to their caseload.
Key Responsibilities:
- Provide oversight to all in-home caregivers, ensuring high-quality care and adherence to NC State Regulations.
- Perform routine in-home supervisory visits to monitor caregiver performance and provide feedback.
- Mentor and guide caregivers within their assigned region, promoting best practices and clinical excellence.
- Develop and deliver education to caregivers, staff, clients, and other parties on clinical issues, including disease processes and hands-on tasks.
In-Home Nursing:
The RN Care Manager will also provide in-home nursing services, including wound care, medication management, and other hands-on tasks. This requires a high level of comfort and confidence in providing these services.
Additional Responsibilities:
- Community outreach and support to ensure seamless care coordination.
- Care management services, including intake assessments and consultations.
- Performance management, ensuring high-quality care and staff performance.
Requirements:
- Registered Nurse (RN) degree required.
- Excellent communication and interpersonal skills, with the ability to interface effectively with clients, families, and healthcare professionals.
- Ability to work in a team environment, promoting collaboration and clinical excellence.
- Valid driver's license and proof of current auto insurance required.
Preferred Qualifications:
A minimum of two (2) years of experience in nursing, with a strong background in care management and clinical expertise.
