Nurse Care Specialist

Affirmed Home Care is seeking a skilled Licensed Practical Nurse to provide high-quality nursing care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Responsibilities may include:
- Providing "Best Practice" nursing care to patients in their homes
- Checking vital signs and documenting in a complete and confidential manner
- Evaluating patient wellbeing and reporting any needs to the Director of Clinical Nursing or emergency services
- Providing personal care and meals
- Administering medications and treatments as prescribed
- Providing emotional support to clients and families in times of crisis
Requirements:
- Valid LPN license
- Annual physical within one year
- PPD or Quantiferon within one year or chest x-ray within five years
- MMR labs
- COVID vaccine card with booster
