Job Title: Registered Nurse Care Manager

UW Medicine Primary Care at Ballard is seeking a Registered Nurse Care Manager to support the growth of our patient population and manage complex patient cases involving chronically ill patients.

Job Summary:

The Registered Nurse Care Manager will work closely with Providers to provide care coordination to patients, assess psychosocial needs, and implement strategies to ensure patients receive the appropriate levels of care.

Responsibilities:
  • Assess patients' medical history, disease process, medications, laboratory data, and manage therapy per provider's treatment plan.
  • Communicate plan of care in a timely manner to provider, patient, and family to establish mutual goals.
  • Educate and coach patients/families on disease states, behavior modification, and ED and resource utilization.
  • Apply nursing best practices to identify and prioritize healthcare triage interventions that are appropriate, effective, and that reduce the risk of adverse outcomes.
Requirements:
  • Current license to practice as a registered nurse in the State of Washington.
