Job Summary

The Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care services to patients in inpatient and outpatient settings. This includes assessing medication therapy, providing clinical interventions, and collaborating with multidisciplinary care providers to ensure safe and effective medication use.

Key Responsibilities

* Assess and monitor medication therapy for patients in inpatient and outpatient settings
* Provide clinical interventions to optimize medication use and patient outcomes
* Collaborate with care providers to ensure medication use is aligned with patient needs and evidence-based practices
* Participate in quality improvement initiatives to enhance patient care and safety

Requirements

* Doctor of Pharmacy (Pharm.D.) degree from an accredited College of Pharmacy
* Current Oregon Pharmacist license
* American Heart Association Basic Life Support (BLS) Certification
* American Heart Association Advanced Cardiovascular Life Support (ACLS) Certification (obtained within 6 months of hire)
* Board Certified Pharmacotherapy Specialist (BCPS) certification through Board of Pharmacy Specialties (BPS) or other relevant BPS certification

Preferred Qualifications

* Completion of ASHP accredited residency or minimum of 3 years of relevant hospital experience
* Strong communication and collaboration skills
* Ability to work in a fast-paced environment and prioritize tasks effectively
