The North Colorado Health Alliance is seeking a Community Health Worker to join our team. As a Community Health Worker, you will be working with individuals and families in the community, providing them with support and guidance as they navigate the healthcare system.

In this role, you will be responsible for connecting individuals with local resources and services, such as counseling and medication-assisted treatment (MAT). You will also provide education and outreach to communities affected by addiction, helping to reduce stigma and promote healthy behaviors.

Salary: $45,000 - $55,000 per year, depending on experience

Job Description:

Key Responsibilities:

  • Connect individuals with local resources and services, including counseling and MAT
  • Provide education and outreach to communities affected by addiction
  • Support individuals in achieving their recovery goals through regular check-ins and goal setting
  • Collaborate with other organizations and stakeholders to promote community health and well-being

Required Skills and Qualifications:

  • Bachelor's degree in a related field, such as social work or public health
  • Minimum 2 years of experience working in the field of addiction recovery
  • Strong communication and interpersonal skills
  • Able to maintain confidentiality and handle sensitive information
  • Familiarity with local resources and services
