Key Responsibilities- Meet with new patients in their homes to orient them about services, identify needs, and suggest healthcare professionals and services.
- Provide comprehensive care to hospital at home patients, including laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries.
- Perform physical examinations and conduct support groups on physical and emotional health topics.
- Act as a resource person for patients, families, and healthcare professionals, providing guidance on health-related decisions and planning.
- Maintain accurate patient records, compile statistics, and conduct research as needed.
- Collaborate with staff to identify patient needs and develop plans to meet those needs.
- Follow patients in hospital at home settings for continuity of care.
- Participate in committees dealing with healthcare and patient care.
- Master's Degree in Science of Nursing or Physician Assistant Studies or related field.
- Minimum 3 years of advanced practice provider experience.
- Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas.
- Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants.
- American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification.
- American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification.
- Satisfactory references from employers and/or professional peers.
- Satisfactory employment verification.
- Satisfactory licensing verification.
- Satisfactory education verification.
- Satisfactory criminal background check.
- Satisfactory drug screen.
This role operates in a patient's home medical office setting, requiring travel by automobile and exposure to various environmental conditions.
Equipment UsedThis role routinely uses standard office equipment and medical equipment, including blood pressure cuffs, thermometers, and stethoscopes.
Physical RequirementsThis role requires regular standing, walking, reaching, twisting, stooping, and kneeling, as well as the ability to use hands and fingers to handle or feel and to talk or hear.
