As a key member of our EMPOWERED Mobile Health Unit (MHU) Initiative, you will play a vital role in addressing the unique challenges faced by underserved communities in Northern Nevada.

The successful candidate will be responsible for providing comprehensive, wraparound services to pregnant and postpartum individuals affected by or at risk for Opioid Use Disorder (OUD). This will involve conducting health assessments, developing personalized care plans, and facilitating community resource referrals and care coordination.

We are looking for an individual with a strong commitment to community service, excellent interpersonal skills, and a deep understanding of healthcare knowledge. Key Responsibilities:

  • Conduct health assessments and develop personalized care plans for clients
  • Provide direct support, education, and assistance to pregnant and postpartum individuals affected by OUD
  • Facilitate community resource referrals and coordinate care between patients, healthcare providers, and community organizations
  • Assist in community outreach, engagement, and awareness activities to promote MHU services
  • Collaborate closely with healthcare providers, social workers, and community partners to ensure integrated care
  • Utilize mobile health technologies and telehealth platforms to support service delivery
  • Participate in ongoing training and professional development activities

Requirements:

  • High school diploma and CHW certification required
  • Minimum of 1 year of experience in community health, social services, or related field
  • Knowledge of opioid use disorders, women's health issues, and challenges facing rural healthcare
  • Demonstrated cultural competence and sensitivity to the needs of vulnerable communities
  • Proficiency in using mobile health technologies, basic computer skills, and familiarity with electronic health records
  • Valid driver's license and ability to travel extensively and safely within Northern Nevada
  • Bilingual skills (English/Spanish) highly desirable
