Job Summary:We are seeking a Per Diem Social Worker III to join our team. As a key member of our multidisciplinary treatment team, you will conduct psychosocial assessments, develop effective treatment plans, and provide direct clinical services to patients and their families.
Essential Functions:- Conduct comprehensive psychosocial assessments of patients and develop individualized treatment plans that prioritize patient safety and well-being.
- Participate in regular treatment team meetings to discuss patient progress, treatment planning, and discharge planning.
- Deliver direct clinical services, including individual and group therapy sessions, as needed.
- Conduct intakes, coordinate placements within facility programs, and assist with developing clinically relevant activities.
- Communicate with family members to ensure they understand and support their loved ones' treatment plans.
- Manage a caseload, maintain accurate records, and ensure compliance with facility and accreditation standards.
- Facilitate education, support groups, and referrals as requested.
- Provide guidance and supervision to social work interns completing field placements.
We require a Licensed Clinical Social Worker (LCSW) and prefer a Master's degree in Social Work (MSW). As an equal opportunity employer, we welcome applications from qualified candidates.
