About the Position

Mendocino Community Health Clinic seeks a skilled Community Care Case Manager to join our team. As a key member of our staff, you will play a vital role in supporting patients on their journey toward sustainable long-term improvements.

Job Description

This position involves engaging and assisting patients in breaking down social barriers to enhance whole-person health and wellness. You will collaborate closely with clients, healthcare teams, and the CalAIM Supervisor to facilitate better health outcomes and empower clients to actively participate in their recovery.

Responsibilities
  • Advocate for patients to break down social barriers and enhance whole-person health and wellness.
  • Guide clients in achieving self-directed, lasting changes that align with their values and care plans.
  • Support clients in managing substance use disorders, chronic conditions, and other health challenges.
Requirements and Qualifications
  • Pursuing or holding a certification in addiction counseling, Peer Support Specialist, or Health Coach; OR experience in nursing, social work, or public policy.
  • Knowledge of group process, psychosocial concepts, support systems, stages of change, Motivational Interviewing, ethical standards, and medical and behavioral health care systems.
  • Valid California Driver's License and auto liability insurance.
Estimated Salary and Benefits

We offer an estimated annual salary of $65,000 - $85,000 based on qualifications and experience. Our benefits package includes medical, dental, and vision insurance, paid time off, life insurance, 401k match, flexible spending account, and more.
