Job Summary:

As a Licensed Practical Nurse (LPN) with Carle Health, you will play a vital role in providing high-quality nursing care to patients in their homes. Under the direction of a Registered Nurse (RN), you will assist in the provision of patient care, ensuring the delivery of safe and effective services.

Key Responsibilities:

• Assist RNs in the provision of patient care, including administering medications, performing wound care, and providing vital signs.
• Conduct client assessments, complete visit documentation, and implement nursing interventions as needed.
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
• Participate in client case reports, provide routine primary client reports, and maintain accurate and timely documentation.

Requirements:

• Current LPN licensure in good standing in the State of Illinois.
• Basic Life Support (BLS) certification within 30 days of hire.
• Ability to work independently, prioritize tasks, and maintain confidentiality.
• Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
