LPN - Home Care Specialist

As a Licensed Practical Nurse (LPN) with Carle Health, you will play a vital role in providing high-quality nursing care to patients in their homes. Under the direction of a Registered Nurse (RN), you will assist in the provision of skilled care to patients, ensuring their comfort, safety, and well-being.
Responsibilities:
Conduct thorough client assessments and complete visit documentation according to Carle Health policy and regulatory requirements.
Implement nursing interventions to meet client needs and changing conditions, in accordance with Carle Health policy and procedures.
Communicate effectively with RNs, clients, and caregivers to ensure seamless care coordination.
Participate in client case reports and team meetings to share knowledge and best practices.
Maintain accurate and complete clinical records, adhering to Carle Health policies and regulatory standards.
Requirements:
Current Licensed Practical Nurse (LPN) licensure in good standing in the State of Illinois.
Basic Life Support (BLS) certification within 30 days of hire.
Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
Familiarity with home care software and email use.
