Job Title: Critical Care Nurse - Home Health Specialist

Primary Responsibilities:

  1. Provide comprehensive assessments of patients in their homes: Conduct thorough evaluations to identify patient needs and develop personalized care plans.
  2. Conduct hybrid tele and in-field assessments: Utilize technology to conduct remote assessments while also visiting patients in their homes to provide hands-on care.
  3. Manage multiple complex tasks simultaneously: Prioritize tasks, manage time effectively, and maintain accurate records to ensure seamless patient care.
  4. Review health status questions and make evaluations and determinations based on UAS criteria: Analyze patient data to determine the most effective course of action and make recommendations for care.
  5. Communicate effectively with patients and families: Provide clear, concise information about patient care, address concerns, and facilitate open communication.
  6. Work independently and collaboratively as part of a multidisciplinary team: Collaborate with other healthcare professionals to ensure comprehensive patient care and achieve optimal outcomes.

Requirements:

  • Minimum of two (2) years of experience in a home health setting within the last ten (10) years.
  • Current RN Licensure in the state.
  • Basic Life Support (BLS) certification required before the start date.
