Job Summary

The Institute for Community Living, Inc. is seeking a skilled Registered Nurse Assessor to join our team. As a key member of our Adult Home Supported Housing Project, you will play a critical role in streamlining the assessment and placement of residents into supported housing.

Key Responsibilities
  • Collaborate with the Social Worker Assessor and multi-disciplinary team members to ensure a smooth transition of residents into independent living arrangements.
  • Conduct comprehensive nursing assessments of residents' physical health needs and develop individualized care plans.
  • Implement nursing strategies and interventions according to established care plans and evaluate residents' responses for achievement of realistic client-centered outcomes.
  • Document residents' responses to care plans and maintain accurate records.
  • Prepare reports in a timely and accurate manner as assigned.
Requirements
  • NYS license and valid, current registration as a Registered Professional Nurse.
  • At least 5 years of experience providing direct services to individuals with mental illness.
  • Strong assessment and analytical skills, with a commitment to person-directed care.
  • Excellent interpersonal, organizational, time management, decision making, and problem-solving skills.
  • Ability to maintain confidentiality and follow direction.
