The role of the Ambulatory Care Pharmacist involves delivering comprehensive pharmaceutical care services in an outpatient setting. The pharmacist is dedicated to promoting high-quality, evidence-based, and cost-effective pharmaceutical care within the assigned clinical areas.
Key Responsibilities:
- Adhere to Kaiser Permanente's policies, procedures, and relevant legal regulations.
- Conduct thorough medication history assessments upon referrals from healthcare providers.
- Develop and implement treatment plans based on patient diagnoses, which may encompass medication management and lifestyle changes.
- Identify and resolve medication-related issues, including untreated conditions, inappropriate drug choices, and adverse drug reactions.
- Adjust and monitor drug therapies in accordance with established protocols, utilizing patient interviews and medical records.
- Coordinate laboratory monitoring related to drug therapies, ensuring accurate interpretation of results.
- Evaluate patient outcomes and compliance, aiming to optimize medication use and minimize adverse drug reactions.
- Provide education to patients regarding their health conditions, medication adherence, and potential side effects.
- Engage in continuous improvement of pharmaceutical care services.
- Deliver educational sessions to healthcare providers and staff through various formats.
- Conduct comprehensive medication reviews and reconciliation processes.
- Utilize pharmacy information systems and maintain accurate documentation in patient records.
- Foster a collaborative environment that supports labor-management partnerships.
- Perform additional duties as necessary.
Qualifications:
Experience:
- A minimum of 1200 hours of experience in collaborative drug therapy management within the last five years, or completion of an ASHP Accredited PGY-1 residency.
Education:
- A degree in Pharmacy that meets the educational requirements for licensure.
Licensure and Certification:
- California Pharmacist License required upon hire.
- Basic Life Support certification within three months of hire.
- National Provider Identifier required upon hire.
Additional Requirements:
- Successful completion of a standardized competency assessment, with exceptions for current Ambulatory Care Pharmacists.
- Knowledge: In-depth understanding of pharmacological principles, medical terminology, disease management, and pharmacy service regulations.
- Experience with aseptic techniques and preparation of sterile products.
- Familiarity with current drug information sources and medical literature.
- Skills: Ability to extract relevant information for patient management, effective communication, analytical thinking, and teamwork within a multidisciplinary setting.
Preferred Qualifications:
- Doctorate of Pharmacy (Pharm.D.)
- Completion of a PGY-2 clinical pharmacy specialty residency or fellowship.
- Board certification in a relevant practice area.
- Advanced Practice Pharmacist License.
