Job Overview

Position Summary

The Patient Care Assistant is responsible for delivering both direct and indirect care to residents under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). This role involves assisting residents with daily living activities, ensuring personal hygiene, comfort, and maintaining a safe and tidy environment for a designated group of residents. The ideal candidate will interact with all stakeholders (co-workers, patients, families, physicians, vendors, etc.) with professionalism and a commitment to exceptional customer service, embodying a spirit of teamwork and respect for the organization.

Key Responsibilities

  1. Facilitates the admission process for new residents.
  2. Assists residents during admission, transfer, and discharge procedures.
  3. Provides morning care, which may include bathing, oral hygiene, grooming, dressing, and general upkeep of the living space.
  4. Delivers evening care including washing hands and faces, oral hygiene, and ensuring residents are comfortable for the night.
  5. Supports RN in executing treatment plans.
  6. Offers general aide support such as repositioning residents, assisting with mobility, and utilizing special equipment as needed.
  7. Records vital signs including temperature, pulse, respiration, weight, and blood pressure.
  8. Follows facility policies and procedures diligently.
  9. Engages residents in social activities to promote interaction.
  10. Ensures proper positioning of residents to prevent discomfort and injury.
  11. Monitors skin condition and reports any concerns to nursing staff.
  12. Maintains cleanliness and organization of resident clothing and personal items.
  13. Checks and organizes linen supplies, ensuring availability and cleanliness.
  14. Treats residents and their families with dignity and respect.
  15. Assists residents to appointments as directed.
  16. Prepares residents for meals and assists with dining as necessary.
  17. Distributes refreshments and ensures residents have access to hydration.
  18. Provides care for residents in isolation as required.
  19. Responds promptly to residents' needs and anticipates requirements.
  20. Supports residents in managing their personal belongings.
  21. Transports residents to various departments as needed.
  22. Reports any changes in residents' conditions or behaviors to appropriate staff.
  23. Participates in interdisciplinary care meetings.
  24. Demonstrates the ability to follow both oral and written instructions and collaborates effectively with team members.
  25. Shows willingness to learn and develop skills in basic nursing procedures.
  26. Maintains confidentiality of resident information while fostering positive relationships.
  27. Engages in ongoing education to enhance skills and knowledge.
  28. Maintains personal health to ensure consistent attendance at work.
  29. Exhibits a genuine interest in caring for geriatric and disabled individuals.

Qualifications

  1. High School diploma or equivalent.
  2. Current Certified Nursing Assistant (CNA) certification in the relevant state.
  3. Basic Cardiac Life Support certification required within a specified timeframe of employment.
