Nurse Practitioner for Transformative Primary Care

We are seeking a highly skilled Nurse Practitioner to join our team in El Paso, TX. As a key member of our primary care team, you will play a vital role in delivering exceptional patient care and contributing to the advancement of innovative healthcare solutions.
With competitive salaries ranging from $120,000 to $160,000 per year, depending on experience, you will have the opportunity to grow your career while making a meaningful impact in the lives of our patients.
Job DescriptionIn this role, you will be responsible for providing comprehensive in-home assessments, performing annual health evaluations, and delivering cost-effective, quality care to assigned patients. You will also work collaboratively with our multidisciplinary team to develop, implement, and refine home health processes.
Your key responsibilities will include:
- Performing comprehensive in-home assessments to identify potential gaps in care and assist the primary care team in better managing chronic diseases
- Reviewing current symptoms and identifying diagnosis to inform care management and active medical management of treatment
- Completing physical examinations, medication reviews, vital checks, lab draws, wound care, and depression screenings
