JOB TITLE: Advanced Practice Professionals - Heart and Vascular Institute

DEPARTMENT: Heart and Vascular Institute

REPORTS TO: Administrative Director of Heart and Vascular Institute, Faculty Supervisor, Section of CT Surgery, Chairman of HVI Surgery

FLSA STATUS: Exempt

ROLE SUMMARY: The Advanced Practice Professional (APP) is equipped with the coordination and clinical expertise necessary for the holistic management of patients undergoing vascular surgery. This includes comprehensive assessments that encompass biological, social, and psychological factors. The APP collaborates with various medical specialty teams to formulate a thorough care plan, reviews it with the clinical team, directs patient care, performs diagnostic and therapeutic procedures, and oversees discharge planning as needed. Additionally, the APP plays a vital role in educating residents and medical/nursing students.

ESSENTIAL QUALIFICATIONS FOR NURSE PRACTITIONER:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  1. Graduation from a CCNE accredited nursing program with a master's degree in nursing.
  2. National Certification from a recognized APRN Certification Center.
  3. Licensure or eligibility for licensure as a Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  4. Department Specific: APRNs seeking privileges as First Assists must obtain RNFA certification.
  5. BLS Certification is mandatory, with additional Advanced Life Support training as determined by the department.

EXPERIENCE:

  1. Preferred experience in acute care or vascular surgery.
  2. New graduates are eligible for consideration.

PREFERRED QUALIFICATIONS:

  1. APRN preferred, but FNPs will also be considered.

ESSENTIAL QUALIFICATIONS FOR PHYSICIAN ASSISTANT:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

  1. Bachelor's Degree as a Medical/Surgical Physician Assistant is required.
  2. Graduation from an ARC-PA approved Physician Assistant training program is required.
  3. Certification from the National Commission for Certification of Physician Assistants is required.
  4. Must be licensed or eligible for licensure by the West Virginia Board of Medicine.
  5. BLS Certification is required, with additional Advanced Life Support training as determined by the department.

  1. Bachelor's Degree as a Physician Assistant or a master's degree as a Physician Assistant is required.

CORE RESPONSIBILITIES: The following duties are indicative of the general nature of work performed by individuals in this role. They are not intended to be an exhaustive list of all responsibilities and duties:

  1. Oversee comprehensive and tailored care for vascular surgery patients and their families during hospitalization, immediate post-discharge, and in outpatient settings.
  2. Conduct daily rounds; perform consultations and pre-procedure evaluations.
  3. Execute integrated assessments.
  4. Engage in perioperative management of vascular surgery patients, which may include first assisting in the operating room.
  5. In collaboration with vascular surgery faculty or by adhering to established clinical guidelines, order necessary tests or consultations.
  6. Conduct and communicate results of diagnostic tests in accordance with clinical guidelines and scope of practice.
  7. Consult and communicate with clinic and hospital staff regarding abnormal findings in patient evaluations.
  8. Perform thorough clinical evaluations and document findings in the medical record.
  9. Facilitate patient discharge by coordinating necessary services and tests for outpatient follow-up appointments.
  10. Develop individualized care plans for vascular surgery patients, incorporating education, discharge planning, and referrals to other healthcare team members as necessary.
  11. Demonstrate expertise in caring for patients across various age groups based on educational experience and certification.
  12. Identify complex or unstable vascular surgery patient care situations and facilitate appropriate interventions.
  13. Recognize emergency situations and initiate effective emergency care.
  14. Evaluate vascular surgery patients and coordinate follow-up procedures and tests in outpatient settings.
  15. Assist in the collection of outcome measures as applicable.
  16. Exhibit critical thinking and diagnostic reasoning skills in clinical decision-making, addressing the individual needs of patients and families.
  17. Utilize equipment safely and effectively.
  18. Base decisions and actions on relevant scientific principles, established standards of care, and research findings.
  19. Employ teaching/learning theories and appropriate strategies to educate nursing staff, hospital staff, students, patients, families, and the community.
  20. Foster an environment conducive to learning by responding to patient/family preferences and readiness to learn.
  21. Continuously update knowledge throughout the year.
  22. Complete all identified clinical competencies.
  23. Serve as a resource for staff caring for vascular surgery patients.
  24. Provide educational programs to professional and paraprofessional personnel in areas of expertise.
  25. Participate in community programs and distribute literature as appropriate.
  26. Document care plans, interventions, and evaluations clearly and concisely for every patient visit in accordance with hospital policies and compliance standards.
  27. Collaborate with other leaders and staff to achieve patient outcome-based improvements in care.
  28. Lead the development and implementation of changes in vascular surgery practices that positively impact patient outcomes.
  29. Work collaboratively with medical and nursing staff to facilitate problem-solving and provide professional support.
  30. Engage in interdisciplinary approaches to patient care, program development, and education, serving as a professional role model.
  31. Promote and practice innovation in the expanded role of care delivery to vascular surgery patients and families throughout hospitalization and outpatient settings.
  32. Participate in committees and task forces, providing leadership as needed.
  33. Document hours and submit billing reports as appropriate.
  34. Review and interpret vascular surgery data from various sources, recommending and implementing necessary changes.
  35. Participate in clinical trials and research studies with the Principal Investigator.
  36. Assist vascular surgery faculty, administrators, and directors in collecting outcome measures.
  37. Perform other duties/projects as assigned.
