MDS Coordinator Position

Job Title: MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Complete Care at Glendale. As a key member of our nursing staff, you will play a vital role in ensuring the highest quality of care for our residents.

Responsibilities:
  • Monitor and guide the completion of resident assessments in a timely manner.
  • Supervise the inputting of MDS assessments and transmit them to the state for validation.
  • Schedule and facilitate care plan conferences on a quarterly and annual basis.
  • Perform other duties as assigned by the Director of Nursing and Administrator.
Requirements:
  • Strong working knowledge of Case-Mix, Federal Medicare PPS process, and medical reimbursement.
  • Ability to work independently or as part of a team.
  • Computer knowledge and previous long term care experience preferred.
  • Active RN license in CT required.
