  • Ensure infection control policies are followed, and implement procedures to prevent the spread of infections.
  • Order supplies, medications, and equipment as needed to ensure seamless delivery of care.
  • Collaborate with interdisciplinary teams to achieve client goals and outcomes.

Responsibilities

  • Deliver high-quality patient care, prioritizing client needs and preferences.
  • Conduct thorough assessments, develop care plans, and implement interventions to meet client goals.
  • Coordinate with healthcare professionals to ensure continuity of care and optimize client outcomes.
  • Communicate effectively with clients, families, and healthcare providers to ensure understanding and compliance with care plans.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements

  • Current licensure as an LPN/LVN in good standing from the state(s) of practice.
  • Valid CPR certification.
  • Ability to work in a home-like setting, adapting to changing environments and situations.
  • Physical stamina to perform duties, including bending, lifting, and standing for extended periods.
  • Demonstrated knowledge of individual service plans, client rights, and health needs.

Estimated Salary: $65,000 - $85,000 per year, depending on experience and location.
