Job Summary
- Provide individualized care to clients in their homes, promoting dignity, independence, and overall well-being.
- Monitor vital signs, report abnormal findings, and administer medications as prescribed.
- Document nursing actions, communicate with family and healthcare providers, and maintain accurate records.
- Ensure infection control policies are followed, and implement procedures to prevent the spread of infections.
- Order supplies, medications, and equipment as needed to ensure seamless delivery of care.
- Collaborate with interdisciplinary teams to achieve client goals and outcomes.
Responsibilities
- Deliver high-quality patient care, prioritizing client needs and preferences.
- Conduct thorough assessments, develop care plans, and implement interventions to meet client goals.
- Coordinate with healthcare professionals to ensure continuity of care and optimize client outcomes.
- Communicate effectively with clients, families, and healthcare providers to ensure understanding and compliance with care plans.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
- Current licensure as an LPN/LVN in good standing from the state(s) of practice.
- Valid CPR certification.
- Ability to work in a home-like setting, adapting to changing environments and situations.
- Physical stamina to perform duties, including bending, lifting, and standing for extended periods.
- Demonstrated knowledge of individual service plans, client rights, and health needs.
Estimated Salary: $65,000 - $85,000 per year, depending on experience and location.
