Key Responsibilities
  • Develop and maintain relationships with residents and their families, ensuring a consistent and caring approach to care.
  • Assess and evaluate resident clinical conditions, documenting findings and notifying providers as necessary.
  • Update and modify care plans to reflect changes in resident care and service needs.
  • Supervise and lead care associates, promoting a positive and supportive work environment.
  • Administer medications accurately and timely, following established protocols.
  • Participate in meetings related to resident outcomes and care, contributing to continuous quality improvement.
  • Respond to resident, family, or responsible party inquiries in a timely and professional manner.
  • Participate in clinical indicator reviews, including falls, wounds, nutrition, and antipsychotic use.
Requirements
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • A minimum of one year experience in assisted living, home health, or long-term care preferred.
  • A passion for caring for seniors and a strong advocate for resident rights.
  • Experience with electronic medical records preferred.
  • Pa LPN License.
