We are seeking a highly skilled Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team at LiveWell Choice, LLC. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our members in a nursing home setting.
Job Summary:The Advanced Practice Provider will be responsible for:
- Providing collaborative primary care and services to a panel of Longevity Health members
- Executing and delivering Longevity Health Model of Care activities
- Developing and implementing advanced care planning
- Facilitating health plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS, and STARs quality gap closure
- Serving as a trusted advisor to members, loved ones, and nursing home staff
- Delivering effective and frequent communication and coordination with members' PCPs, loved ones, and care team members
To be successful in this role, you will need:
- A state unrestricted licensed Advanced Practice Nurse or Physician's Assistant
- National Board certification as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant
- DEA License
- An active national provider number (NPI) and an active Medicare provider number
- Valid state driver's license with a good driving record and proof of automobile insurance
- Auto liability insurance declaration page with minimum limits of $100,000/$300,000 for bodily injury and $100,000 for property damage with combined single limit of $300,000
We prefer candidates with:
- Minimum of 6 months experience as a Nurse Practitioner or Physician's Assistant
- Experience working in the nursing home environment
- Computer/Technology proficiency
- The ability to engage and communicate effectively with physicians, patients, family members, and care team members
