About the Role:

As a Registered Nurse Care Coordinator, you will:

  • Assess existing or incoming referral patient/family strengths and needs.
  • Develop a family-centered plan of care as a member of the interdisciplinary team.
  • Manage transitions between settings, caregivers, and providers.
  • Facilitate communication and collaboration amongst the health care team.
  • Attend and coordinate inpatient care conferences for home-based patients.
  • Assist in educating others regarding the role of care coordination/case management.
Requirements:

To be successful in this role, you will need:

  • A BSN degree.
  • Current licensure as a registered nurse in the state of Ohio.
  • A valid Ohio driver's license and proof of auto insurance.
  • Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certification desirable.
