About the Role:As a Registered Nurse Care Coordinator, you will:
- Assess existing or incoming referral patient/family strengths and needs.
- Develop a family-centered plan of care as a member of the interdisciplinary team.
- Manage transitions between settings, caregivers, and providers.
- Facilitate communication and collaboration amongst the health care team.
- Attend and coordinate inpatient care conferences for home-based patients.
- Assist in educating others regarding the role of care coordination/case management.
To be successful in this role, you will need:
- A BSN degree.
- Current licensure as a registered nurse in the state of Ohio.
- A valid Ohio driver's license and proof of auto insurance.
- Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certification desirable.
