Job Summary

We are seeking a skilled Clinical Intake Coordinator to join our team at Hackensack Meridian Health. As a key member of our home care services team, you will be responsible for processing incoming referrals, ensuring clinical completeness, and providing exceptional customer service.

Key Responsibilities
  • Process all assigned incoming Central Intake referrals in a timely and efficient manner
  • Ensure all referrals are clinically complete, accurate, and meet operational requirements
  • Liaise with physician offices, external facilities, and hospitals to provide customer service and support home care referrals
  • Act as a liaison to referral sources, providing information on programs and services available to meet customer needs
  • Submit team productivity reports and work collaboratively with other team leaders as needed
Requirements
  • Graduate of an RN or LPN program
  • Two to four years of recent clinical experience
  • Proficient in Google platform (Google Docs, Google Sheets, etc.)
  • NJ State Professional Registered Nurse License or Licensed Practical Nurse
