Job Title: Home Health Registered Nurse

We are seeking a compassionate and dedicated Registered Nurse to join our team at Boundless Home Health. As a Home Health Registered Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities:
  • Conduct thorough assessments of patients' health status and develop individualized care plans in consultation with the healthcare team.
  • Administer medications, treatments, and wound care as prescribed by the physician.
  • Monitor patients' vital signs and symptoms and report any changes or concerns to the physician promptly.
  • Educate patients and their families on proper healthcare strategies, disease prevention, and medication management.
  • Collaborate with other healthcare professionals to provide holistic care to patients.
  • Document all patient care activities accurately and timely in electronic health records.
  • Provide emotional support and encouragement to patients and their families.
Qualifications & Requirements:
  • Graduate of an accredited school of nursing with a valid and current Registered Nurse (RN) license.
  • Licensed by the Texas State Board of Nurse Examiners.
  • Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related field.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Proficient in using electronic health records and other healthcare software.
