We are seeking a compassionate and dedicated Home Health Licensed Vocational Nurse (LVN) to provide high-quality nursing care to patients in the comfort of their homes.

Responsibilities:
  • Conduct thorough assessments of patients' health status and develop individualized care plans in consultation with the healthcare team.
  • Administer medications, treatments, and wound care as prescribed by the physician.
  • Monitor patients' vital signs and symptoms and report any changes or concerns to the physician promptly.
  • Educate patients and their families on proper healthcare strategies, disease prevention, and medication management.
  • Collaborate with other healthcare professionals, such as RNs and therapists, to provide holistic care to patients.
  • Document all patient care activities accurately and timely in electronic health records.
  • Provide emotional support and encouragement to patients and their families.
  • Ensure compliance with all applicable laws, regulations, and agency policies, such as OSHA and CDC guidelines for Universal Precautions in home health care.
Qualifications & Requirements:
  • Graduate of an accredited school of nursing with a valid and current Licensed Vocational Nurse (LVN) license by the Texas State Board of Nurse Examiners.
  • Preferred minimum of 1 year of clinical nursing experience, preferably in home health or a related field, with a minimum of 2 hours of general, critical care, or emergency hospital experience.
  • Strong clinical assessment and critical thinking skills.
  • Skills and knowledge with infusion therapy and wound care are a bonus but not required.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Proficient in using electronic health records and other healthcare software.
  • Compassionate, empathetic, and patient-centered approach to care.
  • Reliable transportation and valid driver's license.
  • Basic Life Support (BLS) certification.
