Job Description:

Crestone Wellness, LLC is seeking an experienced Patient Care Coordinator to join our team. The successful candidate will provide exceptional client care and support, working closely with staff to manage daily schedules and coordinate client needs.

About the Job:

The Patient Care Coordinator will be responsible for directly caring for clients, including safely transporting them to events and appointments, aiding other departments with client care, and ensuring clients' awareness of individual and group sessions.

Duties and Responsibilities:

  • Maintain knowledge of Crestone Wellness Policy and Procedures
  • Act as a Liaison between patients and staff, relaying pertinent patient information
  • Supervise daily patient operations and ensure sensitive care
  • Support staff in enforcing rules and guidelines outlined in the Resident Handbook
  • Encourage participation in group sessions, individual therapy, and outside appointments

Requirements:

  • Ability to read and comprehend simple instructions and memos
  • Effective communication skills for presenting information to customers and colleagues
  • Familiarity with basic software and computer navigation
  • Able to apply common sense understanding to carry out instructions

Compensation:

We offer a competitive salary range of $50,000 - $60,000 per year, depending on experience.
