Job Title: LPN (Licensed Practical Nurse)

We are seeking a skilled LPN to join our team at Five Star Care (2). As a Licensed Practical Nurse, you will play a vital role in providing high-quality care to our residents.

Responsibilities:
  • Administer medications and treatments to residents
  • Assist with admissions, transfers, and discharges of residents
  • Document resident care services in medical records and department records
  • Educate residents and family members on treatment, rehabilitation, and care plans
  • Adhere to infection control policies and facility procedures
  • Consult with physicians on resident care plans and notify them of changes
  • Maintain resident confidentiality and privacy
  • Ensure quality of care by adhering to DOH standards and facility policies
  • Collaborate with the interdisciplinary team to resolve resident issues
Requirements:
  • Nursing degree from an accredited college or university, or completion of an approved LPN program
  • Experience in long-term care preferred
  • Excellent communication and teamwork skills
