Job Summary

We are seeking a dedicated and compassionate LVN to join our Kindful Health team as a Hospice Field Nurse. As a key member of our care team, you will work closely with our RN Case Manager to provide exceptional care to patients and their families.

Responsibilities
  • Provide high-quality, patient-centered care in a hospice setting
  • Collaborate with the RN Case Manager and other healthcare professionals to develop and implement individualized care plans
  • Conduct regular assessments and evaluations to ensure patient needs are met
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in interdisciplinary team meetings to discuss patient progress and develop strategies for improvement
Requirements
  • Graduate of an approved school of professional nursing
  • At least 2 years of nursing experience in the hospice service line in the state of Texas
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Compassionate and empathetic approach to patient care
