Key Responsibilities- Coordinate Patient Care: Work closely with healthcare providers to develop and implement individualized care plans, ensuring patients receive comprehensive and coordinated care.
- Education and Support: Provide patients and their families with education and support to facilitate understanding of heart failure and treatment plans, promoting self-management and empowerment.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes and experiences, contributing to the development of best practices and evidence-based care.
- Communication and Collaboration: Facilitate clear communication between referring physicians, care providers, and other healthcare team members to ensure timely and effective treatment planning.
- Program Development: Collaborate with the care team to develop and implement programs that promote patient engagement, education, and support, such as the Meds to Beds program and 30-day post-discharge tele-monitoring services.
- Education: Bachelor of Science Degree in Nursing and three (3) years of clinical experience in Cardiology OR Associate Degree in Nursing with five (5) years of clinical experience in Cardiology.
- Licensure: Registered Nurse license in the state of Georgia.
- Certification: Must achieve Heart Failure Certification (CHFN) through American Association of Heart Failure Nurses Certification Board (AAHFN-CB) within twelve (12) months of hire.
- Knowledge and Skills: Working knowledge of service line and strong foundation in patient support programs.
- Heart Failure Certification: Heart Failure Certification (CHFN) through American Association of Heart Failure Nurses Certification Board (AAHFN-CB).
