Healthcare Transition Specialist

We are seeking a Healthcare Transition Specialist to join our team. As a key member of the multidisciplinary team, you will play a critical role in facilitating patient transitions, working closely with healthcare professionals to ensure continuity of care.
The estimated salary for this role is $70,000 - $90,000 per year, depending on experience and qualifications. A valid RN license in the State of Arizona or an LMSW/LCSW is required, along with a Bachelor's or Master's degree in Nursing or Social Work. One to two years of healthcare experience is preferred.
You will be responsible for conducting assessments, developing transitional plans, and collaborating with healthcare teams to ensure optimal patient outcomes. Timely documentation and compliance with state and federal regulations will also be your responsibility.
