Key Responsibilities:

What We Offer:
  • A competitive salary range of $75,000 - $90,000 per year.

Requirements:
  • Bachelor's degree in nursing, social work, or a related field required; Master's degree preferred.
  • Minimum 2 years of experience in care management or a related field; certification in case management (CCMC) a plus.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders.
  • Ability to prioritize tasks, manage multiple projects, and meet deadlines in a fast-paced environment.
