Job Summary

We are seeking a highly skilled and experienced pharmacist to join our team at Riverside Health System. As a hospital pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.

Key Responsibilities
  • Supervise and perform compounding, packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation.
  • Review, validate, and assess patient-specific medication orders, monitoring patient profiles to ensure appropriate dosage and therapy.
  • Establish an environment of safety and assure safe practices for patients, recognizing and implementing safe medication practices and following policies and procedures that comply with regulatory agency regulations.
  • Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, including IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management, and weight-based dosing.
Requirements
  • Doctoral Degree, Pharmacy (Required)
  • 1 year prior pharmacy experience in area of practice related to the position (Preferred)
  • 1 year PGY1 Residency (Preferred)
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 30 Days (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Preferred)
  • Board Certified Pharmacy Specialties (BCPS) - Board of Pharmacy Specialties (Preferred)
