Job Title: Licensed Practical Nurse LPN - Patient Care Specialist

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Connecticut Institute for Communities, Inc. As a Patient Care Specialist, you will work alongside healthcare professionals to provide high-quality care to our patients.

Responsibilities:

  • Collect and document patient information to inform healthcare decisions.
  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor and record vital signs, medical history, and symptoms.
  • Assist with physical exams and medical procedures as needed and within licensure.
  • Provide education to patients and families on healthcare topics.
  • Maintain accurate and timely documentation of patient care.
  • Collaborate with healthcare teams to ensure comprehensive patient care.
  • Triage patient calls using established protocols to determine appropriate care.
  • Provide patient education on after-hours care, triage, and referral services.
  • Communicate with specialists to obtain letters and results, and assist with setting up home therapies as needed.

Requirements:

  • High School Diploma or GED
  • Current LPN license in the state of Connecticut
  • Previous experience in pediatrics preferred
  • Strong communication and interpersonal skills; bilingual strongly encouraged to apply
  • Ability to work in a fast-paced environment
  • Attention to detail and ability to multitask
  • Understanding of medical terminology and nursing protocols
  • Experience with electronic medical records required; eClinical Works preferred
