We are seeking a skilled Social Worker to join our team at Goddard Riverside Community Center. As a Social Worker, you will work creatively and effectively in the community to engage individuals living on the street through outreach by utilizing a harm reduction approach.
Job Summary:The Social Work Case Manager will serve as a case manager for adults clinically complex adults experiencing street homelessness, with the goal of placement into transitional and permanent housing. The position works collaboratively across teams and organizations to achieve this goal.
Responsibilities:- Provide program support regarding clinical assessment of clients and need for hospitalization, coordinate 9.58 as needed
- Provide program support for SPOA applications to obtain mental health wraparound services to those that meet criteria
- Perform intake, assessment, service planning, advocacy, referral and escort services
- Maintain client case records and enter data in computer database using handheld and desktop computers
- Provide follow-up case management for clients placed in housing, to promote retention and document outcomes
- Attend staff meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement
- Interface with various community agencies, funders and stakeholders relevant to our client population
- MSW degree required within 6 months of hire, LMSW required within 9 months of hire
- Understanding of housing first and harm reduction models preferred
- Valid driver's license and a driving record that meets the agency's auto insurance guidelines (including no suspensions within the last 3 years)
