As a Primary Therapist-Hamilton Per Diem, you will be part of a dynamic team that collaborates with top professionals across all disciplines to provide high-quality clinical services.

Key Responsibilities
  • Provide comprehensive clinical services, including clinical triage, psychosocial evaluations, individual, group, and family treatment, treatment plan development, and coordination.
  • Document each treatment session in accordance with Princeton House documentation standards.
  • Coordinate the development and implementation of the multidisciplinary treatment plan and complete treatment plan reviews within established time frames.
  • Attend daily treatment rounds, providing input to and being receptive to input from other members of the treatment team.
  • Identify changes in patients' clinical status and communicate to the appropriate member of the treatment team.
  • Perform case management with internal and external entities for smooth transitions across the continuum of care.
  • Coordinate discharge planning and communicate the plan clearly to patients, families, and all other providers involved.

  • L-LAC or LSW or PS (Required)
